Peru Visa

• Visa is required
• Submit your valid, signed U.S. passport, must have at least six months remaining validity beyond the entry date and have one blank visa page
• 2 Fully completed and signed Visa Application Forms (Section 3 on each application form must include a signature, thumb print and photograph)

• Copy of travel itinerary or airline tickets
• A business letter of responsibility typewritten on your company letterhead addressed to:
Consulate General of Peru in Los Angeles
The letter must explain all of the following:
• Purpose of travel • Business reference to be visited
• Guaranteeing financial support and return transportation to the U.S.
• This letter must be signed by a company officer other than the applicant.
